Steps:
View Patient Record
1. View a patient's record by utilizing the search bar at the top of the homepage.
2. Enter a patient’s name, email, or MPI to find and select a patient record.
3. After selecting the record, the patient record appears, displaying patient information (Patient ID, Birth Date, Phone, Email address, and Active Related Patient).
Manage Patient Demographics
1. Edit patient demographics by clicking on the pencil icon to the right of the name.
Note: This only applies if your clinic is not integrated with an EMR.
Add & Manage Assignments
1. View any current assignments. The assignments can be filtered by “Type” to see just Modules or Forms. The list can be sorted by Status, Participants, Assigned On, and Next Steps.
2. Choose “Add Assignment" to add a new assignment.
Note: If the patient has multiple active episodes of care, select which episode the assignment should be chosen via a pop-up modal.

Add & Manage Episodes
An Episode of Care is a grouping of treatment, education, Forms, Related Patients, and all the Care Team information related to achieving a single outcome for a patient (ex: Egg Freezing, IVF). Patients can go through multiple episodes throughout a treatment journey. New episodes are typically created for new or changing partners (related patients) or new or changing treatment types.
Episodes in this tab are listed with the following information: “Primary Patient”, “Related Patients”, “Start-End” Dates, and “Status”.
1. Choose “New Episode" to add a new episode.
2. Confirm when the pop-up appears asking “Are you sure you want to add a new Episode of Care for PATIENT NAME?” by selecting the green button, “Create New Episode”.
3. Add assignments to the new episode
Add & Manage Related Patients
Related patients are partners of patients or individuals associated with the patient’s treatment.
The related patients are listed by name, MPI, Date of Birth, Phone, and Email.
1. Choose “Add Related Patient" to add a related patient.
2. If there are multiple active episodes, select the episode from the drop-down of active episodes under “Which Episode do you want to assign Related Patients to?”. Once the episode is chosen, select “Confirm”.
Add & Manage Care Team
The care team is composed of members of the clinic involved in the patient’s care journey. The care team members are listed by name and role (e.g., doctor, nurse, clinical reviewers).
1. Choose “Add Care Team Member” to add a care team member.
2. If there are multiple episodes of care, select the episode from the drop-down of active episodes under “Which Episode do you want to assign Care Team member(s) to?”.
3. Once the episode is chosen, select “Confirm".
