When your clinic makes ID verification (IDV) required to sign a form, you will be prompted to complete the authentication.
What do I do?
The steps below should be followed to complete the ID verification.

Select the “Sign” task in the checklist, and then select “Sign Forms” to be redirected to the ID verification.

Under “Let’s verify your identity”, click “I agree” to be directed to a screen to select the ID type that is preferred to be used to complete the ID verification process (e.g., Driver’s license, Passport, State ID card). If you prefer to use a different form of ID, “My ID isn’t listed” can be selected to enter the ID information.
- Once the ID type is chosen, give consent to the collection of your personal information. This consent is required to proceed with ID Verification. The privacy policies can be reviewed by selecting the blue hyperlinks on this page.
Note: If you do not consent, contact your clinic directly with concerns and ask for an alternative.
- On the following screen you have the option to continue the ID Verification process on your mobile device. By continuing on your mobile device you will be able to take a photo of your ID.
To continue on your mobile device you can:
- Enter your mobile phone number, this will send you a link via SMS
- Choose the blue hyperlink to scan a QR code
You also have the option to Upload a file from your desktop instead. If you do not see this option available - your clinic may not have this option enabled for patients.

For mobile ID upload, you will first be asked to allow access to your device's camera.
You will then be asked to take a photo of the chosen ID of the front and back. You can then choose to Redo or Upload the file selected. Your ID upload will then be processed, this may take a few seconds.
- Once your ID is verified, the form will automatically unlock in your browser allowing you to review and sign the assigned forms.
If the ID verification fails through the automatic process outlined above, then the ID will need to be verified by a clinic staff member manually. EngagedMD is not able to assist with the ID verification process as a clinic staff member needs to review it. If your form is time sensitive, we recommend contacting your clinic to speed up the process.
